Referred by a Therapist
We welcome referrals from PTs and OTs. They spend so much time with their patients, so their insight is invaluable in helping us understand a child's needs and goals.
Please complete our Therapist Referral Form and include as much detail as possible. Once received, our team will work directly with the child's primary care provider to obtain the necessary referral.
We believe in collaborative care, so we always keep our referring therapists informed throughout the entire orthotic process!
Referred by a Primary Care Provider
A referral directly from the child's primary care provider is often the fastest way to begin the orthotic process. We encourage providers to include as much information as possible, including the patient's medical history, demographics, and most recent visit notes.
If needed, our team can provide a referral request form to the provider's office by fax. We love partnering with other providers to help children achieve their mobility goals!
