Welcome to Little Motion Co! To help us provide the best care possible, we ask that all new families complete our Intake Form before their first visit.

Please complete this form prior to your child’s assessment. It includes important information about your child’s medical history, mobility, and current orthotic needs. Having this information in advance allows us to prepare for your visit and ensure a smooth, efficient appointment.

Referral Form for Providers

We welcome as much information as you can provide! While our referral form includes key details, feel free to include clinic notes, recent evaluations, therapy goals, or anything else that helps us better understand the child’s needs. The more context we have, the better we can collaborate and create the right orthotic solution from the start. Please fax the completed form & any other pertinent documents to (405) 583-7553.

Frequently Asked Questions

Q: What population do you serve?
A: We specialize in pediatric orthotic care for children from birth through 21 years of age.

Q: What types of orthotics do you provide?
A: We offer both off-the-shelf and custom orthotics for most upper extremity, lower extremity, and spinal conditions. Many of our products are casted or measured for in our OKC office and then purchased from a third-party supplier. However, we do fabricate some of our personal designs in-house. If you have a question about whether we offer a specific type of orthotic, just give us a call!

Q: Do I need a referral?
A: Yes. We require a referral from your child’s doctor (such as a pediatrician, specialist, or primary care provider) before we can begin evaluation and treatment. We gladly accept referrals from physical therapists, occupational therapists, nurses, and other healthcare providers as part of a collaborative care approach. However, please note that a referral from a physician is still required for us to proceed with orthotic services, especially when billing insurance.

Q: Do you take insurance?
A:  While our insurance contracts are still in progress, we do plan to be in-network with Medicaid, BCBS of OK, and Health Choice in the next few months. We are actively working to expand our contracts. Superbills can also be provided if your plan allows for out-of-network reimbursement.

Q: What if I don't have insurance coverage?
A: We offer transparent self-pay pricing and payment plans when needed. Please contact us to discuss your options — we want to ensure every child gets the care they need.

Q: How do I prepare my child for the visit?
A: We recommend bringing: 1) your child's most commonly worn shoes, 2) any braces or orthotics your child currently uses, and 3) a favorite toy to help your kiddo stay comfortable. We strive to make visits fun, friendly, and stress-free!

Q: How do I schedule an appointment for my child?
A: Call us at (405) 237-9154 or email us at admin@littlemotionco.com. Once we receive your child's referral, we will contact you to get scheduled!

Attendance & Cancellation Policy

At Little Motion Co., we are committed to providing high-quality, personalized care to each child we serve. In order to maintain efficient scheduling and respect the time of our clients and staff, we kindly ask that you follow our attendance and cancellation guidelines:

 

Appointments:

Please arrive on time for your scheduled appointment. Late arrivals may result in shortened visits to avoid delays for other patients.

If you are running more than 15 minutes late, we may need to reschedule your appointment.

 

Cancellations & Rescheduling:

We require at least 24 hours notice for cancellations or changes to your appointment.

Cancellations with less than 24 hours notice will be considered a no-show.

After two no-shows, we reserve the right to require a non-refundable deposit to schedule future appointments.
